Responsible for supporting and delivering community health improvement projects. In addition to overseeing identified day to day operations of the individual projects and assisting team members in project planning, project execution and grant writing, position also will create consistency in grant management practices. Maintain and enhance existing community partnerships, diversify partnerships to capitalize on unique collaborative opportunities. Develop overall framework for the hospital’s prevention and community facing programs. Consolidate or bundle similar community programs into categories driven by similar desired outcomes or milestones. Responsible for coordinating activities of departmental health improvement projects, monitoring team/departmental tasks to ensure timely completion in accordance with organization and grant regulations.
